Make any correction or changes that are necessary from last years membership information.

Please Print Clearly and Fill out All Lines on the Form.

If you have a Fax number or E-mail address or Website address, please make sure you list that information. Be sure that the date for your Games in 2010 and 2011 is listed correctly, and return to Roberta Goss at the above address as soon as possible.

Also this year we would like to know the attendance count at your Games for 2009 or the average attendance if you hold your games on two or more days. Please see the last line on the membership form for this information.

PLEASE NOTE:
When I receive your check for the 2010 ASGF Dues, I will send you a Receipt of Payment.

You will need this receipt as your proof of membership in ASGF. In order to purchase Games Insurance from Gulfstream Insurance Group, Inc. for 2010 you will need to enclose a copy of the ASGF Receipt of Payment with your check to Gulfstream.

Please note that Gulfstream Insurance Group, Inc.- Insurance renewal deadline date is, March 1, 2010. Cost of Games Insurance through membership in ASGF in 2009 was $1,063. Gulfstream will be mailing out notices somtime in January.
